How can clients add pictures to their Consultation Form?

This guide shows you how to allow clients to upload their own images, such as inspiration or treatment-area images, when completing their Consultation Form.

Adding 'Image Upload' to a Form

To add an option to upload an image to your form template, go to Manager > Consultations > Form Templates, and select the form.

Next, scroll down, select Add Item > Image Upload > Add.

Don't See the 'Add Item' Option?

If you do not see the option to add an item, this is because you are using a Pre-made form, which cannot be edited. You can only edit Custom forms.

To edit a Pre-made form, duplicate it by checking the box beside the form template and selecting Duplicate. You can then edit and publish the 'Custom' copy.

Renaming the Image Upload Section

If you would like to change the heading of the image upload section after adding it, select Actions > Edit > Change the heading > Add.

To finish, click Save.


What Does it Look Like for Clients?

After receiving their consultation form, or if they're filling it out with you before their appointment, clients will see an option to upload images.

They can upload one or multiple images, using an existing image from their device or taking a picture with their device's camera.


Viewing the Images

After a client submits their consultation form, you can view the images directly on the form.

You can also download or delete them here.

If you have the Premium Forms Add-On, you can also view these images directly in a client's profile under Gallery > Client Submitted Images.
